Case Management

Crossroads Rehab has an icare approved Case Management service. 

Case managers play a central role supporting individuals facilitate their treatment, rehabilitation and care needs.

  • Initial Needs Assessment

- Comprehensive assessment of roles and abilities
- Identification of health and rehabilitation needs,
- Clear recommendations and action plans

  • Discharge Planning

- Clear, responsive and consistent communication with the client, family, funding body               and service providers

  • Case Conferencing

- Facilitation of client/family and health professional conferences

  • Client Contact

- Individual client and family/carer communication including guaranteed regular appointments, in person, by phone, email or Skype
